  • SJHealthcare Provider Services Inc offers personal assistance services under state-approved programs such as PHC, CAS, and FC. We provide services through programs designed to support individuals at home. SJHealthcare Provider Services Inc is an independent provider contracted to deliver services under state-approved programs. We do not represent or speak on behalf of any state or federal agency.

  • Medical Requirements

    • Age & Functional Need: Must be 21 years or older and require assistance with at least one activity of daily living (ADL) such as bathing, dressing, eating, or toileting.

    • Medical Condition: The functional limitation must result from a documented medical condition, verified by a healthcare professional.

    • Setting: Services are provided in the person’s home (not in assisted living or nursing facilities).

    • Companionship Care: Friendly and compassionate caregivers to keep loneliness at bay and brighten everyday moments.

    • Personal Care: Assistance with activities of daily living (ADLs), such as bathing, dressing, and grooming, to maintain dignity and comfort. The full list of ADLs include:

    • Specialized Care: Tailored support for individuals living with chronic conditions, disabilities, or memory-related illnesses like Alzheimer’s or dementia.

    • Family Support Services: Relief for family caregivers to help balance the demands of caregiving with personal time and responsibilities.
